New to TVMDL? Complete a New Client Form.
Thanks for trusting TVMDL with your diagnostic testing needs. To ensure we have the correct information needed to begin testing and generate results, all new clients must complete a New Client Form.
Submission Forms
All submissions must be accompanied by a TVMDL Submission Form. Samples submitted without a TVMDL Submission Form may be delayed in testing.
Do you have a large submission? Include the Specimen Import Spreadsheet or Submission Form Supplement.
Regulatory Testing Forms
Clients are strongly encouraged to include a TVMDL Submission Form in addition to any required regulatory forms. This practice ensures we have all the necessary information to match your case to your TVMDL account.
